Cerebral Palsy Association is looking for a Nursing Home Transition (NHT) Coordinator to join our small, but mighty team! The NHT Program assists participants living in a nursing facility to safely transition back into the community to the setting of their choice.
You will be working with nursing facilities located in Chester county, Delaware county, Montgomery county and the surrounding area.
The NHT Coordinator will assess barriers to discharging that nursing home participants face which may include affordable and handicapped accessible housing, criminal records, medical concerns, lack of formal/informal supports, lack of income and necessary identification documents, credit history, funds needed to secure housing, etc.
The NHT Coordinator will assist and support participants with overcoming these barriers so that a safe discharge to the community can take place.
The NHT Coordinator will work closely with the participant, the Managed Care Organization (MCO) with Community Health Choices Service Coordinator, Nursing Home Social Worker, nurse, therapist, Behavioral Health provider, power of attorney, guardian, family members, friends and whomever the participant wishes to have involved toward their goal of re-entering the community.
Our home office is located in Exton Pennsylvania.

Responsibilities:
Working with the MCO on mutual participants toward their goal of transitioning back into the community Adhering to all MCO NHT policies and procedures Maintaining a caseload of Nursing Home Transition Participants Knowing participants on a personal basis and being aware of their strengths and plans to achieve their desired lifestyle Empowering participants toward being the leaders of their self-directed plans Educating participants about the services available to them to meet their unique needs Assisting participants with enrolling with Home and Community Based Services and other formal supports needed in the community upon discharge when applicable Assisting participants with identifying and securing safe housing that is within the participant's budget Assisting participants with obtaining essential household items needed for the nursing home transition and set up of these items on discharge day Job Requirements:
Bachelor's degree in Social Work or another related field Must have transportation Proof of COVID vaccination Flexibility, professionalism, and being detail-oriented are a must Must be willing to work in a fast pace environment and flexible to work with last minute changes as they occur Experience with discharge planning and/or Home and Community Based services preferred Work from home flexibility is available EOE Job Type:
Full-time Pay:
$43,215.
57 - $47,645.
