The Dietitian plans, develops, implements and maintains programs and systems of nutritional care; ensures the resident nutritional needs are met and documented to fulfill the requirements of all federal, state and local regulations; advises the General Manager and Chef Manager in their facility, in the areas of nutrition, dietetics, and menu coordination.
Nutritional care plans will meet the needs and maximize the quality of life for the Wesley Enhanced Living residents.
Essential
Responsibilities:
Performs assessments on all new health center admissions, routinely on all health center residents and as needed for Care conference meetings.
Develops and implements a nutritional plan of care based on assessment of nutritional needs as part of the resident's total plan of care.
Ensures appropriate documentation of nutritional assessment including but not limited to pertinent lab values, diet or weight changes, nutrition history, any previously identified nutrition-related issues, and recommended intervention in the resident's chart and care plan.
Reviews and monitors the documentation of others regarding nutritional concerns and responds appropriately.
Documents discharge summaries.
Works with the interdisciplinary team to provide appropriate nutrition-related counseling to residents, families and caregivers.
Monitors all resident's weights over a six, three and one month period, and reports the findings in weekly, multidisciplinary weight meetings.
Maintains communication and effective relationships with nursing services, residents, and families in relations to the nutritional goals and/or nutritional issues or concerns.
Provides nutritional instruction to residents and caregivers as needed or as ordered by the physician and completes other nutritional consults and supporting documentation as assigned.
Nutritional instruction can include but is not limited to dietary plans, food selections, adapting plans to the individual's illness and prognosis as indicated.
Instruction may also include education of assisted and independent living residents as necessary.
Provides follow-up counseling as needed.
Visits all residents upon admission and as needed for food preferences and nutritional issues and dietary concerns.
Attends resident care plan meetings and case management meetings as scheduled.
Demonstrates knowledge of and oversight of food production system, sanitation and menu review to assure compliance with regulatory requirements.
Assures that all services and programs are in compliance with federal, state and/or local laws and statutes within their facility.
Performs other duties as required.
QualificationsRelated
Experience:
No previous experience in nutrition management of Independent/Assisted Living or Skilled Nursing geriatric patients required.
Basic knowledge of dietary regulations from the PA Dept of Health for skilled nursing home residents as well as charting and record keeping.
Ability to work independently to complete assessments and plans of care in the required time frame.
Demonstrates ability to work as part of the interdisciplinary team and make meaningful contributions to the development of the plan of care.
Education or Degree Requirements:
Bachelors degree in Nutrition or related field.
Licenses, Certifications, Registrations:
Current Pennsylvania State Dietitian/Nutritionist License.
Active Registered Dietitian with the American Dietetic Association.
